Challange
A large acute and community healthcare provider faced rising pressure from avoidable care home admissions, delayed clinical decision-making, and limited specialist access for vulnerable residents. Care homes relied heavily on hospital-based escalation due to lack of timely clinical support. This created strain on emergency services, increased hospital admissions, and reduced continuity of care for residents. Leadership recognised the need for a redesigned care model, one that leveraged digital technology to provide real-time clinical support while strengthening integration between care homes and hospital teams.
What we did
Axsys led the end-to-end design and implementation of a region-wide care home support model. We developed the operating model and business case, defining governance, clinical pathways, performance metrics, and funding structures to support sustainable adoption. Working alongside clinical leaders and an external technology partner, we oversaw the deployment of telemedicine capability at scale, ensuring system design aligned with real-world workflows and frontline practice. Beyond technology implementation, we restructured workforce arrangements, strengthened accountability frameworks, and embedded structured change and adoption planning, moving the programme from concept to embedded system-wide delivery.
