Challange
A UK NHS provider had developed a new technology enabled model of care designed to support patients safely at home, combining remote monitoring, digital coordination, and multi-disciplinary working. While the strategic vision was clear, operational teams lacked alignment, clarity of roles, and the readiness to deliver the new approach at scale.
What we did
Axsys led the organisation from design to delivery, shaping the conditions for change and building the capabilities needed to implement the new model of care. We worked closely with senior leaders and frontline teams to establish governance, align delivery structures, and introduce digital workflows that enabled safe and effective care at home. Using a structured readiness and adoption approach, we targeted the behaviours and barriers that mattered, strengthening engagement, embedding new ways of working, and accelerating uptake of tech-enabled care.
